Maybe wrong flash type or corrupt image.

If .BIN ripped diret from card then they are correct size - points to flash card being wrong type thus too small as last block would wrap back to top of address space destroying the bank info.
Too bad you have single slot QS. Otherwise could test my theory by placing "bad written card" in A and an SRAM card with good bank that uses the card samples in B. If patches on SRAM work then just bank info was corrupt.
