the problem you stated looks to me like the MIDI-transmission is not 100 % error-free.
This thing with notes sustaining indefinitely happens, when a note-off event gets garbled by a bit-error during transmission. So when you release a key the synth receives no note-off command then and the key stays turned on.
Also, I have experienced a similar thing: Notes would mutate sometimes to aftertouch (and anything else, if you look closely).

Solution:
1) Check if there is an IRQ conflict with your MIDI card's MPU interrupt (easier said than done - the MIDI interface should use an own interrupt NR that no other device uses, if possible)
2) Try another cable (5 pin DIN to 15-pin Sub-D). There is cables that are DC-coupled and there is more expensive cables that are not (which use opto-couplers)
With the former, it is possible to get a ground-loop, also. Maybe try plugging PC and QS to the same wallsocket.
(I'm NOT suggesting to cover one ground pin of the power plug with adhesive tape, because it's dangerous ;-) and I don't think it would help much. )
And if nothing else helps: use another MIDI interface (you get them on an ISA card for a handful of $), but then disable the interface on your sound-card.
