There is a very good single chip preamp by analog devices. Their part number is SSM2017. It's cost around $3 or $4. You'll also need a DPDT switch, (2) 9 volt batteries, 5K ohm pot, mic input jack and output jack and a piece of circuit board. All except the chip can be obtained from Radio Shack.

Page 6 is the circuit you want. Rg is the 5 KOhm pot. To get +9 and -9 volts fron the batteries, you will have to put them in series and center tap them.
The alternative is to go to musiciansfFriend.com and order a mic preamp for approx. $100 .
